Documents required for application for reissue of passport:

• Duly filled-in application form;
  
• Photocopy of old Machine-Readable Passport (MRP). Applicant also needs to present the original MRP at the counter;
• Additional specific documents as evidence for change of any information in the passport;

• In case of the lost passport, a photocopy of general diary of police report;

Service hours: 

On all working days, i.e., Monday to Friday, except holidays:
Receiving of documents and passports: 9.30 am - 12.30 pm
Delivery of documents and passports: 3:30 pm - 4:30 pm

Postal service: 
Applicant can avail the postal service only to receive the passport. He/she will need to send an envelope with own address written on it, old passport (exception for lost passport case), original delivery slip, proof of staying in Portugal, along with postal fee of Euro 4.00.
